You may call the office phone number (260-203-0294) or complete the following form to request a call back to briefly discuss your situation and your interest in counseling or coaching. Please provide a short overview of your concern and the kind of help you are requesting. Note: new client inquiries are returned within one week of submission, typically on Thursday or Friday.

Good Faith Estimate

Under the law, health care consumers who are not using insurance benefits to help pay for their health care have the right to receive a “Good Faith Estimate” explaining how much their medical care will cost. This Good Faith Estimate should include the total expected cost of any non-emergency services or items, including medical tests, prescription drugs, equipment, and hospital fees.

Ask your health care provider to give you a Good Faith Estimate in writing at least 1 business day before your medical service. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule or make a plan to purchase a medical service or item.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

Fees & Insurance

  • Dr. Heath Lisenby is an in-network provider with the following insurance carriers.

    • Aetna

    • United HealthCare - UBH (Optum)

  • Prior to beginning treatment, clients are encouraged to contact their insurance carrier to verify that Heath Lisenby is an in-network provider and to confirm understanding of their deductible and co-pay commitments.

  • Clients may pay for services by cash, check, or with credit card (preferred) and most HSA/Flex Cards. Full payment or deductible/co-pay is due at time of service.

  • Appointments must be cancelled with at least 24-hours notice to avoid a late cancel fee of $50. No-Show Fees are $105.

  • Updated Fee Schedule as of 11/1/2022:

    Regular Fee Billed to Insurance for Mental Health Counseling (In-Network)

    Initial Evaluation $150/45-50 min.

    Individual/Family/Conjoint Therapy $150/55-60 min. (does not include couple therapy)


    Discounted Fee without Insurance (Out-of-Network)

    Evaluation, Individual Therapy, Family/Conjoint Therapy $125/55-60 min. (does not include couple therapy)


    Self-Pay Fees for Couple Therapy or Marriage Counseling (not submitted for insurance reimbursement)

    Most Couple Therapy sessions are for 90 minutes for the first few sessions.

    Insurance Billing Policy regarding Couple Therapy or Marriage Counseling and Family Therapy

    1. Couple Therapy will not be billed to insurance carriers for reimbursement and Super Bills for Couple Therapy will not be provided to the client for the purpose of submitting for insurance reimbursement.

    2. Health Insurance typically covers "Family/Conjoint Therapy" (billing code 90847) which is not the same as Couple Therapy. Family or Conjoint Therapy is used to bring in a partner or family member to facilitate the therapy for an individual client ("identified patient") who has a billable diagnosis, such as depression or anxiety. While it is reasonable and ethical to address family relationship dynamics to help treat the "identified patient's" condition, it is Heath Lisenby, LLC’s position that this does not include ongoing Couple Therapy or Marriage Counseling or Family Therapy.

    3. In Couple Therapy or Family Therapy, neither partner is the "identified patient" and mental health treatment is not the main focus. Both partners are there to work on issues in the relationship that are causing distress and to improve the relationship's functioning and satisfaction.

    4. Concerning Family Therapy, when the main focus of treatment is on the relationship between family members or concerned parties, the above Couple Therapy policy and rates apply to Family Therapy services.
